Transferring Student Registration

MORGAN PARK HIGH SCHOOL REGISTRATION INFORMATION

 

The following documents are required for Registration: (No copies accepted, original documents ONLY)

  • Original Birth certificate 
  • Driver’s license/State ID of Parent/Guardian
  • Residency Verification (Name and address must match the ID).
  • Family Income Form (completed during Registration process)
  • Student Health Information Form (completed during Registration process)
  • School Physical (9th and 12th graders) 

 

Acceptable Residency Verification Documents: 

  • Illinois driver’s license or State of Illinois identification card and one of the following:
  • Current utility bills;
  • Deed;
  • MediPlan/Medicaid Card;
  • Voter registration card;
  • Court documents;
  • Illinois Department of Public Aid card;
  • Illinois state aid check/social security check; and
  • Other identification card issued by a federal or state agency or foreign government consulate, such as a Matricula Consular.

A person who knowingly or willfully presents to the School any false information regarding the residency of a student for the purpose of enabling that student to attend school without the payment of out of district tuition is guilty of a CLASS C MISDEMEANOR (105 ILCS 5/10-20.12b(f).

  • Complete the Online Registration Form FIRST:

This form provides Morgan Park High School with your student’s demographic and contact information to expedite registration.  This form must be completed before arriving to finalize your student's enrollment.

Attention: Morgan Park's Administration reserves the right to conduct a home visit for students with no, or limited, historical residence in Morgan Park's Attendance Area.
